Dr. Elise Kuykendall is known for lighting up any room and her ability to build relationships with her patients. There’s one thing about Dr. Kuykendall that you just can’t miss – her smile! She enjoys helping patients of all ages, in any season of their healthcare journey, with a caring, fully-present approach.

Dr. Kuykendall is thoroughly Oklahoman. She graduated from Norman High School as valedictorian in Norman, OK, before graduating Summa Cum Laude with a B.S. from Southwestern Oklahoma State University. She received her Doctorate of Osteopathic Medicine from Oklahoma State University College of Osteopathic Medicine in Tulsa, OK, where she ranked second in her class. She spent three years in a general surgery residency, believing she wanted to be a surgeon, before having a big change of heart that resulted in pursuing family medicine as her profession. She then completed her residency training at Integris Northwest Family Medicine Residency in Enid, OK, where she served as Chief Resident during her final year.

Dr. Kuykendall celebrates the direct model of care because it allows her to do what she does best – take compassionate and preventive approach for her patients and embrace all aspects of their healthcare needs. She is proud to partner with individuals and families in every facet of their medical journey to be a place of trust and solace. She embraces a preventive approach to care in order to guide both her patients and their families. Due to her time spent in general surgery training, she is well-versed and comfortable in performing simple procedures in the office setting, and is grateful for the opportunity to provide these to her patients. Having a daughter of her own with Down Syndrome, Dr. Kuykendall has a tremendous passion for the care of special needs individuals and enjoys being a pillar of support for these patients and their families.

Her love for her patients runs almost as deep as her love for her family. She and her husband are truly best friends, and have been blessed with four incredible children. They enjoy spending time outdoors, reading books together, and having picnics on the living room floor for movie nights. They also love spending time with their church community at Life.Church.

Dr. Natalie Cochenour is board certified in family medicine, providing preventative medicine and treatment for chronic illness for patients from birth to geriatrics. She graduated Summa cum laude from the University of Central Oklahoma before earning her medical degree from the University of Oklahoma College of Medicine in 2012. After residency, Dr. Cochenour practiced with Integris Medical Group in Yukon from 2015 until she joined Primary Health Partners in 2023.

Introduced to medicine at a young age by watching her pharmacist father care for patients, Dr. Cochenour decided early on that she wanted to be involved not just in implementing a patient’s treatment but in planning it. Now, she finds her greatest reward in sitting down with her patients, getting to know them, and working together to make decisions, educating them at every step of the way. She believes when patients understand the “why” of their condition, as well as the plan to treat it, they can make better choices that align with their needs and values and make the changes necessary for optimal health.

For Dr. Cochenour, providing good medical care is about more than writing a prescription; it’s making care personal and collaborative. Dr. Cochenour firmly believes the future of medicine is to place decisions about treatment back in the hands of patients and their doctors, and she is excited about joining a family of physicians at PHP who share her genuine, authentic approach. As a young mother herself, Dr. Cochenour is especially passionate about women’s health and taking care of young families. Her experience working in the newborn nursery sparked her interest in helping new moms navigate the challenges of first time motherhood, particularly breastfeeding. She looks forward to pursuing further certifications and training in breastfeeding and lactation medicine.

When she’s not in the office, Dr. Cochenour enjoys spending time with her husband and three children. The family is active in their local church, and becomes a house divided on Saturdays in the fall, when Dr. Cochenour cheers for the Sooners while her husband roots for the Kansas State Wildcats.

Dr. Fiorella Avant considers medicine her calling, and loves to hear each patient’s story and to treat them as she would her own family. Born in Costa Rica, she relocated to the US as a young child and spent much of her childhood in California. She moved to El Reno, Oklahoma in high school where she graduated and ultimately put down her Okie roots. She attended Oklahoma State University College of Osteopathic Medicine in Tulsa, where she received her Doctorate of Osteopathic Medicine. She completed her residency at INTEGRIS Northwest Family Medicine Residency Program. After training she joined INTEGRIS, where she has enjoyed taking care of patients in and around the Yukon community for the past 4 years.

Dr. Avant believes in nurturing the doctor/patient relationship and empowering her patients to maintain a healthy lifestyle, from newborns to seniors. She considers her calling a privilege to be part of her patients life and helping them shape their health. She enjoys the variety that comes with working with patients from all walks of life. Dr. Avant is bilingual, Spanish-speaking, and hopes to provide care for the hispanic community in Yukon, Oklahoma and the metro area. She truly enjoys all aspects of family medicine, but takes special interest in preventative care, nutritional counseling, women’s health, and hormone replacement therapy. Dr. Avant loves the direct primary care model because it allows her to spend all the time she needs with her patients.

When she’s not caring for her patients, Dr. Avant enjoys family time with her husband, two daughters and her son. She is passionate about working out and maintaining her own health. She also loves being outdoors, traveling and photography.
